Recently, the emphasis has been on developing techniques for predicting human reliability. Present effort is focused on developing a more academic methodology which applies to practical human-machine systems. However it is desirable to treat the concept of human error with caution and to avoid an approach in which the operator appears to be held solely responsible. The aim of our work is to propose a methodological approach based on the consideration that errors are the combination of conditions in the human-machine system. It is on the total system that attention should be centered.
